Licensed Clinical Social Worker

General Statement of Duties

The Licensed Clinical Social Worker (LCSW) position is to improve and promote overall health within the general patient population, with a focus on addressing the behaviors that are impeding optimum health. The LCSW will work side-by-side with all members of the clinical care team including primary care providers (PCPs) and the nursing staff to enhance preventive and clinical care for health problems that are sensitive to behavioral intervention. The role of the LCSW is to work with individual patients and contribute to systemic change within their primary care that facilitates a multidisciplinary approach both from a treatment and reimbursement standpoint. 

Responsibilities

• Assess, determine and provide a full spectrum of psychosocial support programs to children, adults and families
• Conduct warm handoffs during patient visit with PCP
• Develop individual care plans, support patient efforts to comply with plans, counsel high risk patients
• Consult with physicians and therapy staff in assessing client/family needs in relation to the medical efforts to comply with plans and conduct group education 
• Stay current on all state/federal/local support programs
• Act as a liaison to outside mental and behavioral health agencies, including substance abuse providers, to facilitate further assessment and care when needed.
• Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
• Actively participate in meetings that support the integrated health care model to provide comprehensive care for clients.
• Develop and assist in conducting psychoeducational classes for clients (for example, smoking cessation, stress reduction, mindfulness).
• Identifying problems related to patient services and making recommendations for improvement. 
• Document all activity in medical record as appropriate and in accordance with medical records standards.
• Respond to patient telephone calls as needed.
• Acts as advocate on behalf of patients. 
• Maintains strict patient confidentiality.
• Assists in preparing program materials, policy development research, analysis, and development, and other related program activities.
• Helps monitor, prepare, and disseminate written communications
• Participates and assists in creating and preparing for special events and projects.
• Work collaboratively in a team setting and with outside professionals and area agencies.
• Responsible for maintaining professional, respectful and effective working relationships with staff, clients and community professionals/agencies. 
• Participates in periodic trainings, and satisfactorily completes required training in a timely manner. 
• Adheres to appropriate professional, ethical, and legal standards, and complies with organizational privacy and client confidentiality practices.
• Performs other duties as assigned

Minimum Qualifications

• Licensed Clinical Social Worker (LCSW)
• Master’s degree in psychology, social work or related field
• Valid OR driver's license and vehicle insurance, reliable transportation
• Required Skills/Knowledge/Abilities
• Basic computer knowledge, internet use, MS Office

Preferred Qualifications

• Knowledge of Federally Qualified Health Centers
• Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
• Two years’ experience in an integrated behavioral healthcare setting
• Experience working with low income patients

• Sufficient personal mobility and physical reflexes, with or without reasonable accommodation, which permits the employee to work in a medical and office environment.
• Sufficient personal and physical capabilities, with or without reasonable accommodation, which permits the employee to make presentations to large and small groups.
• Must be able to occasionally lift up to 30 pounds.

Additional Requirements

• Submit to and pass a drug test
• Successfully complete a criminal background check
• Must be able to work beyond normal working hours, including weekends.
• Must be able/willing to attend training sessions or meetings out-of-town on your own time.

Working Conditions

There may be exposure to airborne and blood-borne pathogens, and hazardous materials. This position is not eligible to be a telecommute position.
