MyCare Medical is seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our team serving patients throughout Broward County, Florida.
MyCare Medical is a physician-founded primary care organization focused on delivering high-quality, patient-centered care while improving health outcomes and reducing unnecessary healthcare costs. Our providers and interdisciplinary teams work together to address the whole patient, including medical, behavioral health, and social needs.
The LCSW will provide home- and community-based clinical social work services to patients with complex medical, behavioral, and psychosocial needs. This position will play an important role in addressing Social Determinants of Health (SDOH), providing brief clinical interventions, coordinating community resources, and supporting patients in overcoming barriers that impact their health and quality of life.
The LCSW will work closely with physicians, APPs, RN Case Managers, care coordinators, home health professionals, and other members of the interdisciplinary care team.
Key Responsibilities
Clinical & Psychosocial Assessment
- Conduct comprehensive psychosocial, behavioral health, and SDOH assessments in patients' homes and community settings.
- Identify mental health, emotional, social, environmental, and financial barriers affecting patient health.
- Assess patients for depression, anxiety, social isolation, caregiver stress, abuse/neglect, and other psychosocial concerns.
- Develop individualized, patient-centered intervention plans based on identified needs.
Behavioral Health & Clinical Intervention
- Provide short-term clinical interventions, supportive counseling, crisis intervention, and psychotherapy within the scope of the LCSW license.
- Provide counseling related to adjustment to illness, grief, loss, caregiver stress, depression, anxiety, and life transitions.
- Assist patients in developing coping strategies and improving adherence to treatment plans.
- Identify patients requiring higher levels of behavioral health care and facilitate appropriate referrals.
- Support suicide-risk, safety, and crisis assessments in accordance with MyCare Medical policies and clinical protocols.
Social Determinants of Health & Community Resources
- Identify and address SDOH barriers including:
- Housing instability
- Food insecurity
- Transportation
- Financial hardship
- Social isolation
- Access to medications and healthcare
- Caregiver limitations
- Utilities and basic needs
- Connect patients and families with Broward County community resources, social services, food assistance, transportation programs, housing resources, and other safety-net services.
- Assist eligible patients with accessing Medicare, Medicaid, long-term care, and other available community benefits.
- Develop relationships with community organizations and agencies to strengthen patient referral networks.
- Follow up with patients to determine whether identified resources successfully addressed their needs.
Care Coordination
- Collaborate with PCPs, APPs, RN Case Managers, care coordinators, behavioral health providers, and community organizations.
- Participate in interdisciplinary care planning and case conferences.
- Coordinate referrals and follow-up services to promote continuity of care.
- Assist with transitions of care following hospitalization, rehabilitation, or other changes in level of care.
- Advocate for patients and help ensure their clinical and psychosocial needs are communicated to the care team.
Patient & Family Education
- Educate patients and caregivers regarding available behavioral health and community resources.
- Provide guidance regarding advance care planning, caregiver support, community services, and available benefits when appropriate.
- Encourage patient engagement and participation in individualized care plans.
- Support patients in developing skills to manage psychosocial challenges and improve overall well-being.
Documentation & Compliance
- Maintain accurate, timely documentation of assessments, interventions, care plans, referrals, and patient outcomes.
- Document all services and patient interactions in eClinicalWorks (eCW).
- Maintain compliance with applicable federal and state regulations, professional standards, and MyCare Medical policies.
- Protect patient confidentiality and comply with HIPAA requirements.
Qualifications & Requirements
- Master's degree in Social Work (MSW) from an accredited social work program.
- Active, unrestricted Florida LCSW license required.
- Minimum 2 years of clinical social work experience preferred.
- Experience in home health, hospice, primary care, behavioral health, community health, or case management preferred.
- Home health experience or verifiable clinical experience providing services in patients' homes strongly preferred.
- Demonstrated experience addressing Social Determinants of Health (SDOH) and connecting patients with community resources.
- Experience working with older adults, Medicare/Medicaid populations, medically complex patients, and/or chronically ill populations preferred.
- Strong knowledge of Broward County community resources and social-service networks preferred.
- Experience working as part of an interdisciplinary healthcare team.
- Strong clinical assessment, counseling, crisis-intervention, advocacy, and care-coordination skills.
- Excellent communication, organization, documentation, and time-management skills.
- Proficiency with electronic medical records; eClinicalWorks (eCW) experience preferred.
- Ability to work independently while maintaining strong communication with the clinical team.
- Valid Florida driver's license, reliable personal vehicle, and current automobile insurance required.
- Ability and willingness to travel throughout Broward County for home and community visits.
Compensation
Annual Salary: $85,000–$90,000
Compensation will be based on clinical experience, qualifications, home-based care experience, and demonstrated expertise in SDOH, behavioral health, and community resource navigation.

Pay range and compensation package
Our base salary range for this position is $85,000–$90,000 based on experience.
