Licensed Master Social Worker

The primary function of the Social Worker is to provide coordinated care in the home to patients and perform assessments, analysis, counseling and referrals to meet the needs of the patient and their family.

Essential Functions:

• Assess patients to identify needs.
  


• Develop Plan of Care in consultation with the physician and other care-team members.
  


• Assist other team members in understanding the social and emotional factors related to the health problems.
  


• Utilize and provide patient/family with appropriate resources as needed.
  


• Use effective interpersonal relations and communication skills to work with family members as needed.
  


• Complete appropriate documentation in a timely manner.


• Other duties as assigned.

This position requires occasional travel to our Corpus Christi region, as well as regular travel throughout the Rio Grande Valley for visits. Our service area ranges from Brownsville to Rio Grande City!

Mileage reimbursement is provided.

Job Qualifications:

• Education: Masters Degree in Social Work


• Licensure: LCSW or LMSW required*


• Experience: One (1) year experience in a health care agency or facility.
  


• Skills: Working knowledge of community resources. Good interpersonal skills.
  


• Transportation: Reliable transportation. Valid drivers license and auto liability insurance.
