Job Summary
LNA I
Provides essential support and direct resident care under the supervision of licensed nursing staff. Assists with daily living activities, monitors health conditions, and helps maintain a safe, clean, and supportive environment.
LNA II
Independently provides essential resident care with oversight from licensed nursing staff. Assists with daily living activities, monitors health conditions, mentors new nursing assistants, and contributes to maintaining a safe, clean, and supportive environment.
Approved formal education may substitute for required work experience and approved work experience may substitute for required formal education on a year-for-year basis.
Education/Experience:
High school diploma or equivalent
License/Certification:
NH Nursing Assistant License
Drivers license
Other Requirements:
May be required to work non-traditional hours
Must have physical stamina for lifting, transporting residents, and prolonged standing
Trainee Option:
Candidates without a high school diploma or NH LNA license may be hired in trainee status with adjusted responsibilities.
Experience in long-term care, geriatric care, or similar medical environment
Strong verbal, written, and interpersonal communication skills
CPR and First Aid certification
Trainees must obtain NH LNA license within 6 months (on-site training may be available)

One year of experience as an LNA or medical professional
Competency to mentor new nursing assistants
Satisfactory performance evaluation
Completion of mandatory training

Assist residents with ADLs: bathing, dressing, grooming, toileting, mobility, feeding, and hydration
Support mobility and transport residents to activities, appointments, and events
Follow care plans and document care, behaviors, vital signs, intake/output, and changes in condition
Collect routine lab specimens; report concerns to nursing staff
Maintain supplies, equipment, and PPE; report shortages
Follow infection control and safety protocols
Perform housekeeping tasks in resident rooms and common areas
Prepare residents for medical appointments and consultations
Provide emotional support and maintain positive communication with residents and families
Participate in training, meetings, and in-service programs
Agency/Position-Specific:
Operate facility vehicles for transporting residents or specimens
Implement safety protocols, including restraint procedures when necessary
Accompany residents to activities and provide companionship
Exercise independent judgment in daily resident care
Assist with ADLs and mobility needs; communicate needs outside scope to appropriate staff
Perform treatment procedures within LNA scope; follow medical orders and care plans
Coordinate consulting provider visits and assist health team members
Document care, monitor vital signs, collect specimens, and observe/report changes
Mentor new nursing assistants as assigned
Maintain supplies, inventory, and submit orders
Follow infection control and safety protocols
Maintain cleanliness and complete assigned tasks
Provide emotional support and maintain communication with residents and families
Participate in training, meetings, and committees
