Job Description - LICENSED NURSING ASSISTANT I-III / LMNA

Job Summary

LNA I

Provides essential support and direct care to residents under the direct supervision of licensed nursing staff. Assists with daily living activities, monitors resident health, and helps maintain a safe, clean, comfortable, and engaging environment.

LNA II

Provides essential support and direct care to residents with direction from licensed nursing staff. Works more independently, assists with daily living activities, monitors health conditions, ensures a safe and comfortable environment, and mentors new nursing assistants.

LNA III

Provides essential support and direct care to residents with general oversight from licensed nursing staff. Works independently, assists with daily living activities, monitors health conditions, ensures a safe and enriching environment, mentors/instructs new nursing assistants, and contributes suggestions for improvement in care and procedures.

Your Experience Counts

Each additional year of approved formal education may be substituted for one year of required work experience, and each additional year of approved work experience may be substituted for one year of required formal education.

Minimum Qualifications

LNA I

Education/Experience:

  • High school diploma or equivalent

License/Certification:

  • NH Nursing Assistant License

  • Drivers license

Other Requirements:

  • May require non-traditional work hours

  • Must have physical stamina for prolonged standing, lifting, transferring, and transporting residents/materials

Preferred Qualifications:

  • Experience in long-term or geriatric care

  • Strong communication and interpersonal skills

  • CPR and First Aid certification

Eligibility for Promotion (Level 1 Level 2):

  • One year of experience as an LNA or other medical professional

  • Demonstrated competency to serve as a mentor

  • Satisfactory performance evaluation within past 12 months

  • Completion of mandatory training

  • Supervisor recommendation and approval

LNA II

Education/Experience:

  • Equivalent combination of 1 year of education and experience beyond high school

License/Certification:

  • NH Nursing Assistant License

  • Drivers license

Other Requirements:

  • May require non-traditional work hours

  • Must have physical stamina for prolonged standing, lifting, transferring, and transporting residents/materials

Preferred Qualifications:

  • At least one year of experience as an LNA or medical professional

  • Experience in long-term or geriatric care

  • Strong communication and interpersonal skills

  • CPR and First Aid certification

Medication Certification (In-Band Advancement):

  • Valid NH Medication Nursing Assistant license

  • Assumes responsibility for administering medications

  • Completion of five NH Nursing Boardapproved competencies

  • Satisfactory performance evaluation within past 12 months

  • Completion of mandatory training

  • Supervisor recommendation and approval

LNA III

Education/Experience:

  • Equivalent combination of 3 years of education and experience beyond high school

License/Certification:

  • NH Nursing Assistant license

  • Drivers license

Other Requirements:

  • Completion of five NH-approved competencies

  • May require non-traditional work hours

  • Must possess physical stamina for prolonged standing, lifting, transferring, and transporting residents/materials

Preferred Qualifications:

  • At least three years of LNA experience in long-term or geriatric care

  • Strong communication and interpersonal skills

  • CPR and First Aid certification

Medication Certification (In-Band Advancement):

  • Valid NH Medication Nursing Assistant license

  • Assumes responsibility for administering medications

Responsibilities

LNA I

  • Assists residents with ADLs including bathing, dressing, grooming, toileting, mobility, and nutrition.

  • Uses safe techniques and equipment to promote comfort, dignity, and independence.

  • Follows established care plans and participates in meetings as needed.

  • Documents care, monitors vital signs, and reports changes in condition promptly.

  • Stocks supplies, maintains cleanliness, and follows infection control procedures.

  • Prepares residents for appointments and provides support during medical visits.

  • Demonstrates empathy, communicates effectively, and supports residents and families.

  • Participates in training, meetings, and cooperative teamwork.

LNA II

  • Uses independent judgment to address routine physical and emotional resident needs.

  • Assists with ADLs, promotes independence, and supports mobility and nutrition.

  • Performs treatment procedures within LNA scope and follows care plans and medical orders.

  • Coordinates visits from consulting healthcare professionals.

  • Documents care, vital signs, intake/output, and reports health changes promptly.

  • Mentors new nursing assistants and supports onboarding to policies and procedures.

  • Maintains supply inventory and reports equipment issues.

  • Follows safety and infection control standards and maintains cleanliness.

  • Provides empathetic, professional communication with residents and families.

  • Participates in training, meetings, and cooperative facility operations.

LNA III

  • Mentors and trains new nursing assistants on procedures and standards.

  • Exercises independent judgment to assess and prioritize resident needs.

  • Assists with ADLs, mobility, and nutrition; recommends when additional support is needed.

  • Performs treatment procedures and collaborates on treatment plans for complex needs.

  • Participates in Plan of Care meetings and recommends improvements in care.

  • Coordinates visits with consulting medical professionals.

  • Documents care thoroughly and monitors resident health indicators.

  • Manages inventory and equipment maintenance requests.

  • Ensures safety, cleanliness, and adherence to infection control; suggests practice improvements.

  • Supports residents' emotional and social adjustment; guides therapeutic activities.

  • Communicates professionally with residents, families, and staff.

  • Participates in training, committees, and cooperative teamwork.
