Title: Licensed Nursing Assistant (LNA)

Environment: Hospice House

Location: Concord, NH 

Schedule: Friday-Sunday (7pm-7am)

Pay Range: Up to $26-28/hourly (inclusive of weekend / overnight differentials)

Granite VNA is seeking a compassionate and dependable Licensed Nursing Assistant (LNA) to join our Hospice House team. In this rewarding role, you’ll provide comfort, dignity, and quality-of-life care to residents while supporting their families and working closely with our caring clinical team.

What You’ll Do

• Provide compassionate assistance with daily activities such as bathing, grooming, eating, mobility, transfers, and incontinence care.
• Follow individualized care plans and observe, document, and promptly report changes in residents’ conditions.
• Create a safe, clean, comfortable, and welcoming environment.
• Support residents with cognitive needs and encourage participation in activities that are meaningful to them.
• Communicate professionally and respectfully with residents, families, nurses, and other care team members.
• Work independently while contributing to a supportive team environment.
• Follow safe work practices, agency policies, and LNA scope of practice.

1. High School Diploma/GED, accompanied by current New Hampshire LNA license
