About the Role:

We are seeking a Licensed Practical Nurses to join our team at Care Street Homecare LLC in Flemington, NJ. As an LPN with us, you will have the opportunity to make a meaningful impact on the lives of our clients and their families, while working in a skilled nursing setting.

Responsibilities:

Provide direct patient care, including administering medication and treatments

Assist with the development and implementation of patient care plans

Monitor and report changes in patient symptoms or behavior

Collaborate with other healthcare professionals to ensure high-quality patient care

Document patient care and maintain accurate medical records

Requirements:

Valid LPN license in the state of New Jersey

BLS required

Flu shot recommended

Previous experience in a long term healthcare setting

Compassionate and caring demeanor

Strong communication and interpersonal skills

Ability to work well in a team environment
