Position Overview: Nursing Coordinator (LPN)

We are seeking an experienced and compassionate Nursing Coordinator (Licensed Practical Nurse) to join our team. In this vital role, you will oversee and coordinate nursing care for individuals with developmental disabilities across residential and day service programs. Working closely with our interdisciplinary team, you will help ensure that each individual receives the highest standard of care in a safe, supportive, and respectful environment, while complying with all applicable state and federal regulations.

Key Responsibilities:

• Clinical Oversight: Provide direct nursing care, including administering medications, monitoring vital signs, and carrying out physician-ordered treatments.
• Health Monitoring: Regularly assess and document client health status; identify and report changes in condition to the Registered Nurse (RN) or physician.
• Medication Management: Safely administer medications and treatments; maintain complete and accurate medication administration records.
• Care Coordination: Collaborate with physicians, healthcare providers, families, and support teams to develop and update individualized care plans.
• Patient Advocacy: Serve as an advocate to ensure that each client’s health needs and preferences are recognized and addressed in care planning.
• Training & Support: Provide guidance, training, and support to Direct Support Professionals (DSPs) and other staff on medical procedures, health protocols, and emergency response.
• Documentation: Maintain thorough, accurate, and timely medical records in compliance with HIPAA and organizational policies.
• Health Education: Educate clients and their families on health management, medication, and self-care strategies.
• Emergency Response: Respond promptly to medical emergencies and participate in crisis interventions as needed.
• Compliance: Uphold regulatory standards and agency policies, including state and Medicaid requirements, to ensure high-quality, accredited care.

Salary range

$25 - $30 per hour

 Qualifications:

• Active LPN license in the state of Ohio.  
• Minimum of 1–2 years of nursing experience, ideally within developmental disabilities services, long-term care, or home health settings.  
• Knowledge of the specific health needs and challenges faced by individuals with intellectual and developmental disabilities.  
• Ability to work independently and collaboratively in a multidisciplinary team environment.  
• Strong organizational, time-management, and communication skills.  
• Commitment to delivering compassionate, person-centered care.  
• Current CPR and First Aid certification (or willingness to obtain).
