About the Job

We’re seeking a compassionate, experienced Licensed Practical Nurse (LPN) for our office at Heart Forward MD. You will be pivotal in delivering comprehensive patient care, including assessments, vital sign monitoring, and medication administration. You will play a crucial role in assisting the provider(s) during various diagnostic and therapeutic procedures, contributing to the overall efficiency of the practice. Patient education on cardiovascular health, medication management, and post-procedure care is a vital responsibility, emphasizing the LPN's role in promoting patient well-being.

What you’ll do:

• Answers patients’ inquiries and determines how to assist them.


• Measures and records patients’ vital signs such as height, weight, temperature, blood pressure, pulse, and respiration.


• Works as part of a health care team to assess patient needs, plan and modify care, and implement interventions.


• Assist cardiologist with various diagnostic and therapeutic procedures, such as stress tests, and echocardiograms.


• Administer medications as prescribed by the cardiologist.


• Educate patients on medication management and potential side effects.


• Provide patient education on cardiovascular health, lifestyle modifications, and disease management.


• Maintain accurate and up-to-date patient records, documenting assessments, interventions, and patient responses.


• Ensure compliance with privacy regulations and maintain confidentiality of patient information.


• Collaborate with the healthcare team to ensure coordinated patient care.


• Communicate effectively with patients, families, and other healthcare professionals.


• Ensure the proper functioning and cleanliness of medical equipment used in the practice.


• Participate in training for emergency protocols and procedures as required.


• Participate in quality assurance and improvement initiatives to enhance patient care.


• Attend relevant workshops, seminars, or training sessions as needed.

What you’ll need:

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing


• Current State License as a Licensed Vocational / Practical Nurse


• 1 year of experience in a clinical care setting


• Basic Life Support (BLS) required


• Athena EMR experience preferred

This role will be located in office.  We are a multisite practice with our main hub in Westfield- 17300 Westfield Boulevard, Suite 330, Westfield, IN 46074.

Our satellite location is South Anderson- 2705 Enterprise Drive, Suite 275B, Entrance D, Anderson, IN 46013

Additional Information:

Part time hourly rate starting at $23 per hour.  This role will begin at 24 hours per week.
