Licensed Practical Nurse (LPN) – Home Health Visits

BAYADA Home Health Care is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hudson County Visits team, providing one-on-one skilled nursing visits to adult clients throughout Union County, NJ.

As a BAYADA home health nurse, you'll have the opportunity to build meaningful relationships with your clients while providing high-quality care in the comfort of their homes. You'll work independently while being supported by an experienced interdisciplinary clinical team dedicated to helping clients remain safe and independent at home.

A minimum of one year of recent clinical experience as a Licensed Practical Nurse is required.

Responsibilities

• Provide skilled nursing visits to clients throughout Union County.


• Administer medications, provide wound care, and perform physician-ordered treatments and procedures.


• Monitor client conditions and communicate changes to the Clinical Manager and Client Services Manager.


• Complete nursing assessments as appropriate.


• Follow, execute, and accurately document physician orders.


• Coordinate care with physicians and members of the interdisciplinary care team.


• Educate clients and caregivers to support health, safety, and independence at home.


• Complete timely and accurate documentation using BAYADA's electronic medical record (EMR) system.

Qualifications

• Current New Jersey Licensed Practical Nurse (LPN) license.


• Minimum of one year of recent, verifiable clinical experience as an LPN.


• Graduate of an accredited and approved nursing program.


• Home health experience strongly preferred, but not required.


• Ability to work independently and effectively manage time.


• Strong communication, interpersonal, and organizational skills.


• Experience with electronic medical records (EMR) preferred.


• Reliable transportation and ability to travel to client homes throughout Union County.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation:
$48.00 - $55.00 per point
