What You’ll Need

• NY State LPN License (required)
• Minimum one year of Med/Surg experience in a hospital or long-term care facility
• NY State Driver’s License and reliable transportation
• Good computer skills for electronic documentation
• Compassionate, patient-focused attitude and strong communication skills

What You’ll Do

• Visit patients in their homes to implement and evaluate care plans
• Provide skilled nursing care and emotional support to patients recovering from illness, injury, or surgery
• Document visits electronically and complete all required documentation within 24 - 48 hours
• Collaborate with the Coordinator of Care and other team members to ensure high-quality, personalized care
• Contribute to a positive, patient-centered care experience and support overall health outcomes
