What Does an LPN Do at PDI?

As a Licensed Practical Nurse (LPN) at PDI, you’ll deliver compassionate nursing care and medical oversight for individuals with intellectual and developmental disabilities. You’ll implement health care plans, monitor medications, support medical appointments, ensure proper documentation, and collaborate with other health professionals and direct support staff. This is a hands-on role requiring attention to detail, proactive communication, and a deep commitment to ensuring the health and safety of those we serve.

Key Responsibilities:

  • Administer medications and monitor effectiveness and side effects
  • Implement individualized healthcare plans and crisis plans
  • Accompany individuals to doctor appointments and maintain accurate health documentation
  • Maintain MARs, medication profiles, and track medication variances
  • Monitor therapy plans, assistive devices, dietary plans, and seizure activity
  • Ensure timely follow-up on referrals, lab results, and medical orders
  • Participate in ISP and Circle of Support meetings
  • Report significant health changes or concerns to the Agency RN
  • Educate and support direct care staff on medical needs and processes

Requirements

To be considered for the LPN position at PDI, applicants must meet the following requirements:

Licensure & Education:

  • Current and valid Licensed Practical Nurse (LPN) license in good standing (Must be issued in Tennessee or in a compact state recognized by Tennessee)
  • Proof of education and license required

Special Requirements:

  • Valid Tennessee, Kentucky, or Alabama driver’s license (military exemptions available)
  • Satisfactory criminal background check required
  • Negative Drug test required
  • Satisfactory driving record required
  • Ability to read and follow written instructions
  • Ability to communicate effectively in writing and verbally
  • Availability to attend and successfully pass all required DDA and PDI training courses required
  • PHS and Therap experience preferred

Physical Demands:

  • Ability to lift a minimum of 50 pounds
  • Must be able to regularly stand, walk, sit, use hands, reach, talk, hear, and occasionally stoop, kneel, crouch, or crawl
