Licensed Practical Nurse

Reports to: Assistant Manager 

Duties and Responsibilities:

1. Delivers safe care according to plan of care and as delegated by the RN

• Meets agency productivity standards

• Documents care within time frames

2. Collaborates with health care team

• Reports all findings to RN or designee

• Notifies MD of significant changes

• Seeks supervision appropriately

3. Assumes responsibility for self development

• Maintains competencies 

• Maintains knowledge of new techniques and standards of care

4. Effective communication with Clinical staff

• Coordinators and Clinical Managers to facilitate care planning

5. Assists RN in patient/family communication

• Provides written teaching material to support learning

• Assists in evaluating patient response to education.

6. Documents all communication timely

• Timely communication with teams

7. Assists in growth of the agency

• Accepts other assignments as needed to support agency

8. Supports and encourages patient safety

• Reports all occurrences

• Instructs patient/family in safety measures

• Follows infection control practices.

9. Assists in development of agency processes and procedures

• Provides orientation to new employees

• Cross trains to learn new skills

• Accepts other assignments as requested

Use of personal vehicle is required for home visits.  Driver’s License verification required. 

Job Qualifications/Experience:

• Current NJ LPN license 

• Minimum of one-year experience in acute, subacute, or home care setting

• Experience using an EMR and excellent communication skills.
