What Your Week Looks Like

• Travel within your assigned territory to deliver skilled nursing visits per each patient's physician-ordered plan of care, under RN supervision
• Perform hands-on treatments: wound care, medication administration, injections, IV therapy, and vital signs checks
• Monitor each patient's condition and report changes promptly to the RN or physician
• Educate patients and families on disease management, medications, safety, and self-care so they can manage more independently between visits
• Supervise Home Health Aides on assigned care tasks
• Complete clinical documentation accurately and on time, in line with agency, state, and payer requirements
• Join interdisciplinary case conferences and quality improvement discussions
• Follow strict safety and infection control protocols on every visit

What We're Looking For

• Graduate of an accredited practical or vocational nursing program
• Current, active LPN or LVN license in the state of Oregon
• One year of clinical experience, including one year specifically in home health
• Valid CPR certification, maintained throughout employment
• Valid driver's license and reliable transportation to travel to patient homes across the assigned territory
