Provides nursing care to residents within an assigned work unit under the supervision of a Registered Nurse, following established nursing department guidelines.

Education:
Completion of a recognized school of practical nursing approved by the State Board of Nursing Education and Nurse Registration.
Experience:
No experience required beyond required training.
License/Certification:
Current practical nurse license or temporary permit issued by the Board of Nursing pending licensing examination results.

Each additional year of approved formal education may substitute for one year of required work experience, and vice versa.
Documents nursing care, resident progress, and actions taken.
Administers medications and monitors effects to ensure safe medication management.
Contributes to the nursing process and updates care plans as resident needs change.
Supervises LNAs and conducts rounds to monitor care quality, cleanliness, and safety.
Communicates resident information between shifts and reports unusual findings.
Collaborates with physicians and consulting professionals during resident visits.
Provides direct nursing assistant support, including ADL care, bathing, transfers, and lifting.
Educates residents on care plans and goals.
Communicates with families/guardians regarding resident progress and changes.
Manages resident admissions, transfers, and discharges as directed.
Maintains CPR competency and follows department policies.
Documents nursing care, treatments, and resident progress.

Contributes to care plans and updates them based on resident status.
Supervises LPN I staff and LNAs; conducts rounds to ensure safety and quality of care.
Reports significant observations and facilitates shift-to-shift information transfer.

Provides counseling to residents regarding care plans and goals.
Communicates with families/guardians about resident care and significant changes.
Manages resident admissions, transfers, and discharges.
Provides direct nursing assistant support including bathing, feeding, transfers, and lifting.
Collaborates with healthcare team members to meet resident needs.
Maintains CPR competency and follows department policies.
