As an LPN Visit Nurse with Home Health Care, Inc., that's what your workday is built around.

You'll travel to patients throughout your local area and provide scheduled skilled nursing visits one patient at a time.

Assess what you're seeing.

Work Close to Where You Live

Your territory shouldn't automatically mean the entire Twin Cities.

Patient visits are scheduled within approximately a 35-mile radius of your home, helping create a service area centered around where you live.

You'll spend your day traveling between patients in your local area while receiving mileage reimbursement between patient visits.

• Skilled nursing observations and assessments

• Injections and ordered treatments
  
• Chronic disease monitoring
  
• Patient and caregiver education
  
• Reinforcing physician orders and plans of care
  
• Monitoring for changes in patient condition
  
• Communicating concerns to RN Case Managers and physicians
  
• Collaborating with therapists and other interdisciplinary clinicians
  
• Accurate and timely clinical documentation

What We're Looking For

• Active Minnesota Licensed Practical Nurse (LPN) license
  
• Strong clinical observation and patient-care skills
  
• Excellent communication and organization
  
• Ability to work independently in patients' homes
  
• Compassionate and dependable approach to care
  
• Reliable vehicle, valid driver's license, and current auto insurance
  
• Ability to travel to patients within approximately a 35-mile radius of your home
  
• Home health experience preferred, but not required
