Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and other medical treatments
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Conduct resident assessments, identify care needs, and make recommendations for modifications to care plans as needed
• Maintain accurate and timely documentation of resident care
• Communicate effectively with residents, families, and healthcare providers to ensure seamless coordination of care

Requirements:

Current LN license in good standing
1+ year of experience in a skilled nursing facility or similar healthcare setting
Basic Life Support (BLS) certification
Ability to work flexible hours, including days, evenings, and weekends
