We are seeking a dedicated Licensed Practical Nurse (LPN) with the highest standards of excellence, who seeks the opportunity to work for an employer that’s the leader in patient-focused, team-based hospice care and committed to creating a culture of excellence.
Our LPNs provides nursing care to the terminally ill hospice patient following plan of care (POC) for assigned patients, as needed. You will provide assistance and understanding to the family in the home care situation and in time of bereavement. In addition you will provide nursing care, education, and support to the patient/family and/or caregivers.
Northern Illinois Hospice supports our employees’ wellbeing and regularly benchmarks with other companies in our industry to offer an extensive Total Reward package for this position. The expected base wage range for this role is $28.00 - $37.00. However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Additionally, this role may be eligible for other forms of compensation and benefits.

Requirements
Education: Completion of a formal Practical Nurse Program.
Training and Licensure: Current Practical Nurse Program (LPN) license issued by the Illinois Board of Nursing required.
Knowledge/Skills/Ability: Ability to work independently, make accurate, and at times, quick judgements. Ability to respond appropriately to crisis in a community setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes.
Experience: A minimum of two (2) years of nursing experience which includes basic nursing skills with the administration of medications. Home care experience preferred.
Other: Local residence with ability to respond to on-call within 30 minutes. Meets agency requirements for criminal background check.
