We are seeking a compassionate and skilled Hospice LPN to join our growing team. This role provides direct patient care, supports families, and helps deliver comfortfocused care to individuals at the end of life. Responsibilities
• Provide direct nursing care under the supervision of an RN Case Manager
• Monitor patient symptoms, medication needs, and comfort levels
• Perform routine nursing procedures in accordance with the plan of care
• Document assessments, interventions, and patient status accurately and timely
• Communicate changes in condition to the interdisciplinary hospice team
• Provide education and emotional support to patients and their families
• Participate in team meetings and support overall hospice care goals

Qualifications
• Current LPN license in the state of Kansas
• Hospice or home health experience preferred, but not required
• Strong clinical and communication skills
• Ability to work independently in patient homes or facilities
• Compassionate approach to endoflife care
• Valid driver’s license and reliable transportation
