Licensed Practical Nurse - LPN - Continuous Care nights

Empath Health is currently seeking a Licensed Practical Nurse (LPN) to join our Continuous Care (CC) team.

Schedule 8pm - 8am Week 1 & 2: Tue, Wed, Thu. Coverage area to include Manatee, Charlotte, De Soto, and Sarasota County.

What you do will do:

• Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.


• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.


• Crisis Care nurses are cross-trained to Hospice Houses and Triage to assist with patient care needs when cases are pending.

Requirements:

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.


• Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
