Licensed Practical Nurse (LPN) Home Health

We are looking for compassionate Licensed Practical Nurses (LPNs) who want to make a meaningful impact by providing high-quality, evidence-based care in a home health setting. • Flexible Scheduling – Full-time, part-time, and PRN options available

• Provide skilled nursing care under the direct supervision of a Registered Nurse


• Assist with care planning, implementation, and evaluation


• Perform specialized procedures as directed by an RN


• Educate clients on self-care techniques


• Observe, record, and report patient conditions


• Assist in rehabilitation efforts by monitoring patient mobility and alignment


• Participate in case conferences, in-services, and staff meetings

• Active LPN license in Virginia


• At least one year of LPN experience (home health preferred)


• Current CPR certification


• Reliable transportation, valid driver’s license, and proof of car insurance


• Ability to pass background checks and provide references
