JOB SUMMARY

The Licensed Practical/Vocational Nurse (LPN/LVN) is responsible for delivering skilled nursing care to patients on an intermittent basis in their place of residence. This care is provided under the direction and supervision of a Registered Nurse (RN), in accordance with physician orders and a personalized plan of care. All services are performed in alignment with agency policies and regulatory standards.

QUALIFICATIONS

• Graduate of an approved school of practical or vocational nursing, with an active LPN/LVN license in the practicing state.
• Minimum of one (1) year of nursing experience preferred.
• Commitment to the philosophy and goals of the agency.
• Ability to exercise initiative and make independent judgments as needed.
• Strong communication skills and compassionate approach to patient care.

Requirements

RESPONSIBILITIES

• Follow established agency policies and procedures consistently.
• Implement the nursing care plan tailored to each patient’s needs.
• Provide nursing services, treatments, and diagnostic and preventive procedures as assigned.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Observe and document patient signs and symptoms, reporting any reactions to treatments, including medication reactions and changes in physical or emotional condition, to the physician and RN.
• Educate and counsel patients and their families on nursing care needs and any other related healthcare concerns.
• Collaborate with the RN to evaluate the effectiveness of the LPN/LVN’s nursing services for patients and their families.
• Maintain accurate and complete records of observations, treatments, and patient care activities.
• Participate in medical record audits as assigned to ensure compliance.
• Attend staff meetings, patient care conferences, and in-service training as scheduled.
• Take on-call duties, including nights, weekends, and holidays, as assigned.
• Submit any schedule changes to the Director of Clinical Services or Clinical Manager daily.
• Participate in patient care conferences to discuss the need for involvement of other healthcare professionals, such as physical therapists or speech-language pathologists.
• Prepare clinical and progress notes accurately and promptly.
• Assist the physician and RN with specialized procedures as needed.
• Prepare equipment and materials for treatments and follow aseptic techniques as required.
• Support patients in learning appropriate self-care techniques to promote independence.

WORKING ENVIRONMENT

Primarily works indoors, both in an office setting and in patient homes, with travel required between patient homes.

JOB RELATIONSHIPS

Reports to: Director of Clinical Services, Clinical Manager, or Registered Nurses (RNs)

RISK EXPOSURE

High risk due to potential exposure to infectious diseases and patient interactions in home settings.

LIFTING REQUIREMENTS

The role requires the ability to engage in physical activities essential for patient care, including extended periods of standing and frequent bending, lifting, and heavy lifting on a regular basis.
