What You'll Do

• 
  

  Provide skilled nursing care in a home health setting under physician direction

  
  


• 
  

  Perform in-home visits, including treatments, monitoring, and follow-up care

  
  


• 
  

  Administer medications and treatments within LPN scope of practice

  
  


• 
  

  Observe and report changes in patient condition to the RN and care team

  
  


• 
  

  Educate patients and families on medications, disease management, and safety

  
  


• 
  

  Document visits in Homecare Homebase (HCHB) – training provided

  
  


• 
  

  Collaborate with an interdisciplinary team (RNs, therapists, caregivers)

  
  


• 
  

  Travel locally within Mesa County (Grand Junction, Fruita, Clifton, Palisade)

  
  

What You Bring

• 
  

  Active Colorado LPN license (unrestricted), IV Certification a plus!

  
  


• 
  

  Ability to work independently and manage your schedule in the field

  
  


• 
  

  Strong communication and patient-centered care approach

  
  


• 
  

  Comfort with documentation and basic technology

  
  


• 
  

  Valid driver's license, reliable vehicle, and auto insurance

  
  


• 
  

  Home health or community-based care experience preferred (we will train the right candidate)
