Location: Community Health Center of the North Country - Ogdensburg, NY

Full-Time | Monday–Friday, Daytime Hours | No Nights, Weekends, or Holidays
Pay Range: $18.72 - $25.00 per hour

REPORTS TO: Practice Manager / Clinical Development Manager

We’re seeking a skilled and dedicated Licensed Practical Nurse (LPN) to join our collaborative care team in a welcoming outpatient setting. 

What You’ll Do

• Greet and prepare pediatric and adult patients for visits, including taking vitals and updating medical histories
•  Prepare charts and review records to ensure timely and organized visits 
• Assist providers during exams, procedures, and treatments
• Accurately document patient information in the electronic medical record (EMR)
• Respond to routine patient phone messages, escalating concerns to the RN or provider as needed
• Process prescription refills and coordinate with pharmacies
• Monitor and manage clinic supplies and vendor equipment deliveries
• Support infection control protocols and help maintain a safe, welcoming care environment
• Participate actively in team meetings and quality improvement efforts

Requirements

What You’ll Need

• Current New York State LPN license
   
• Current BLS certification (training can be provided post-hire)
   
• Strong communication, clinical, and organizational skills
   
• Experience with electronic medical records preferred
   
• Commitment to professionalism, teamwork, and confidentiality
