Licensed Practical Nurse LPN /Staff Scheduler / Medical Records

The LPN/Staff Scheduler/Medical Records position is a multifaceted role responsible for ensuring adequate nursing staff coverage, maintaining accurate and organized medical records, and providing direct patient care as a Licensed Practical Nurse (LPN) within a skilled nursing facility (SNF) setting. This role requires a strong understanding of nursing principles, excellent organizational skills, attention to detail, and the ability to work effectively in a fast-paced environment. 

Key Responsibilities: 

Staff Scheduling 

• Develop and maintain nursing staff schedules (RNs, LPNs, CNAs) to ensure appropriate staffing levels 24/7, adhering to facility policies, state regulations, and resident care needs. 
• Manage call-ins, time-off requests, and vacation requests, finding suitable replacements to prevent staffing shortages. 
• Communicate schedule changes effectively to staff. 
• Track and manage employee attendance and tardiness in accordance with facility policies. 
• Collaborate with nursing management to anticipate staffing needs and make proactive adjustments. 
• Maintain accurate records of staff credentials, licenses, and certifications to ensure compliance. 

Medical Records Management 

• Maintain a comprehensive and organized medical records system in compliance with federal, state, and facility regulations (e.g., HIPAA, OBRA). 
• Process new admissions, discharges, and transfers, ensuring all necessary documentation is complete and filed appropriately. 
• Audit medical records for completeness, accuracy, and compliance with documentation standards. 
• Retrieve and distribute medical records as requested by authorized personnel, ensuring confidentiality. 
• Manage record retention and destruction according to established policies. 
• Assist with preparing for surveys and audits by ensuring medical records are readily accessible and organized.
• Maintain electronic health records (EHR) systems, if applicable, ensuring data integrity and user proficiency. 

Licensed Practical Nurse (LPN) Duties 

• Provide direct nursing care to residents as an LPN, including medication administration, wound care, treatments, and vital sign monitoring, as needed to support nursing staff or in emergencies. 
• ? Assess residents' conditions, report changes to the Registered Nurse (RN) or physician, and document observations accurately. 
• Collaborate with the interdisciplinary team to develop and implement individualized care plans. 
• Educate residents and their families on health maintenance and disease prevention. 
• Adhere to all nursing standards of practice, facility policies, and procedures. 
• Participate in resident care conferences as required. 

Administrative and Other Duties 

• Maintain confidentiality of all resident and employee information. 
• Participate in facility meetings, in-service training, and continuing education programs. 
• Assist with administrative tasks as assigned by the DON or Administrator. 
• Promote a positive and collaborative work environment. 
• Adhere to all infection control and safety policies.

Qualifications: 

? Education: Graduate of an accredited school of practical nursing. 

? Licensure: Current, unencumbered LPN license in the state you are applying for.

? Experience: Minimum of 1 years of experience in a skilled nursing facility or long-term care setting preferred, with demonstrated experience in scheduling and/or medical records. 

? Skills: 

? Proficient in medical terminology and documentation. 

? Strong organizational and time management skills. 

? Excellent written and verbal communication skills. 

? Proficiency with computer systems, including scheduling software and Electronic Health Records (EHR) systems. 

? Ability to work independently and as part of a team. 

? Strong attention to detail and accuracy. 

? Ability to handle multiple tasks and prioritize effectively. 

? Knowledge of federal and state regulations pertaining to skilled nursing facilities, including staffing requirements and medical record guidelines. 

? Compassionate and patient-centered approach to care.

Physical Requirements: 

• Ability to sit, stand, walk, bend, and lift up to 50 pounds as needed. 
• Ability to perform nursing duties requiring dexterity and fine motor skills. 
• Visual and auditory acuity sufficient to assess residents and perform job duties. 

Working Conditions: 

? Works primarily in an office setting with frequent interaction with staff and residents. ? May be exposed to infectious diseases, unpleasant sights/odors, and challenging behaviors from residents. 

? Requires flexibility to work occasional evenings, weekends, or holidays as needed for staffing coverage.
