Licensed Practical Nurse LPN Weekend Day Shift

Northeast (Every other weekend) 

Scheduled hours: 24 hours per week

Compensation: $30.00 to $35.00 per hour, depending on experience

Openings: Two openings, one in Northeast DC and one in Southeast DC

Position Summary

Innovative Life Solutions is seeking a dependable and compassionate Licensed Practical Nurse to provide person-centered nursing support to individuals with intellectual and developmental disabilities. The LPN will monitor health needs, administer medications and treatments, maintain accurate documentation, communicate changes promptly, and collaborate with the interdisciplinary team to support safe and consistent care.

This position is assigned to the Saturday and Sunday 8:00 AM to 8:00 PM schedule in Northeast or Southeast Washington, DC. Apply only if you can work the full recurring schedule.

Essential Responsibilities

• Provide nursing services in accordance with each person’s plan of care, physician orders, organizational policies, and applicable requirements.
• Administer medications and treatments safely and document services accurately in the medication administration record and other required records.
• Monitor each person’s health status, identify changes or concerns, and take appropriate action, including timely notification and escalation.
• Complete nursing notes, health assessments, incident documentation, medication reviews, and other required records within established timeframes.
• Coordinate with physicians, pharmacies, hospitals, families, direct support professionals, managers, and other members of the interdisciplinary team.
• Support continuity of care during admissions, discharges, appointments, transitions, and changes in medication or treatment orders.
• Provide guidance to direct support staff regarding health protocols, medication-related concerns, and person-specific nursing instructions within the LPN scope of practice.
• Follow infection-prevention, confidentiality, safety, and emergency-response procedures and report suspected abuse, neglect, exploitation, or other reportable events as required.
• Maintain professional conduct and dependable attendance for the complete assigned shift.

Required Qualifications

• Current, active, and unrestricted Licensed Practical Nurse license authorizing practice in the District of Columbia.
• Current CPR and First Aid certification, or ability to obtain required certification within the organization’s established timeframe.
• Completion of all required DC Department on Disability Services DDS Phase Trainings. Applicants who previously completed the trainings must provide acceptable documentation for verification.
• Ability to successfully complete all required background checks and regulatory clearances as a condition of employment and assignment.
• Ability to satisfy all other required pre-employment, health, credentialing, and regulatory requirements.
• Working knowledge of medication administration, nursing documentation, infection prevention, and recognition of changes in health status.
• Ability to communicate clearly, maintain confidential information, use electronic documentation systems, and work effectively with an interdisciplinary team.
• Reliable transportation and the ability to report consistently to the assigned Washington DC work location.

Preferred Qualifications

Experience supporting people with intellectual or developmental disabilities in residential, community-based, long-term care, behavioral health, or related settings.

Experience with electronic medication administration records and person-centered plans of care.
