About the Job:

We're seeking a Licensed Practical Nurse (LPN), that will serve as a pivotal support to Registered Nurses (RNs) in executing patient care plans. The main duties encompass assisting with specialized procedures, contributing to care plan preparation and evaluation, and addressing the emotional and physical needs of patients. You'll play a key role in patient education, empowering individuals with self-care knowledge. Additionally, you actively engage in fostering teamwork, understanding the roles of other healthcare professionals, and participating in collaborative discussions to optimize patient outcomes. Requirements:
  • Graduation from an accredited School of Practical Nursing.
  • Must be licensed in the State as a Licensed Practical Nurse.
  • One year or more of experience in either a home health agency or hospital setting, with a preference for home health experience.
  • Essential capability to access clients' homes, even if they are not routinely wheelchair accessible.
  • Adequate hearing, eyesight, and physical dexterity required for conducting client assessments and administering care.
  • Physical tasks may include walking, sitting, stooping, standing, and lifting clients, ranging from minimal to maximum weight, as well as turning clients.

Responsiblities:
  • Assists the Registered Nurse in carrying out the plan of care.
  • Assists the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physician's orders.
  • Assists with the preparation, implementation and continuing evaluation of the client care plan.
  • Recognizes and understands the effects of social and economic problems upon clients and provides for the emotional and physical comfort and safety of clients.
  • Helps teach the patient the appropriate self-care techniques.
  • Fosters cooperative effort among personnel by understanding the functions of other persons involved in client care and by active participation in team and staff conferences.
  • Assumes responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on the job learning.
  • Prepares clinical and progress notes (not to include admission).
  • Observes, records, and reports to the appropriate person patient symptoms, reactions and changes.
