Licensed Practical Nurse (LPN) - Value Based Medicine

General Job Summary: The VBM Licensed Practical Nurse will work in a telehealth capacity as part of a care team to improve the execution of quality requirements, increase throughput, improve efficiency, and enhance the overall patient experience.  

(This is a Full time position, Monday to Friday 8 am to 5 pm.  This position will be based at our MSO in SouthPark, with the option to work one day per week remote).

Primary Job Responsibilities/Tasks may include, but not limited to:

• Demonstrates the knowledge and skills necessary to provide care or arrange for the provision of appropriate care, for patients, as pertinent to the scope of nursing practice and specialty of the office setting.
• Will be responsible for providing Medicare Annual Wellness visits to patients over the phone.
• Appropriately and accurately document all patient care/interactions in patient medical record to ensure all quality measures are being met.
• Ensures that all ordered laboratory and diagnostic tests are tracked and logged, and that the provider is presented with all test results.
• May assist with satisfaction of pertinent health maintenance requirements.
• Reports patient's condition and reaction to the provider following procedures, medication administration, or treatments.
• Assists with patient inquiries for prescription refills, appointment scheduling, and referral authorization as needed.
• Participates in electronic documentation of care at the direction of the provider.
• Performs other duties as assigned.

Requirements:

Education and Certifications:

• Certification of Licensed Practical Nurse with current license to practice in NC required.
• Graduate of an Accredited School of practical nursing required.
• Previous medical office experience preferred.
• Maintain American Heart Association or American Red Cross BLS certification.
• Completion of Hepatitis B Vaccine and Flu Vaccine Form will be required.  Candidate may decline vaccination through declination form or may provide record of vaccination from previous employer.
• Completion of TB test will be required.

Experience:

• Minimum of one-year professional nursing experience, clinic experience preferred.

Physical Requirements:

• Work consistently requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
• Must be able to lift and support weight of 35 pounds.
• Ability to concentrate on details.
• Use of computer for long periods of time.
