Licensed Practical / Vocational Nurse (Lpn/Lvn)

Job Description;

POSITION REQUIREMENTS:

1. Current practical/vocational nurse license.
2. Full command, verbal and written, of the English language.
3. Current physical and PPD as per facility requirements.
4. Prefer a minimum of one year of facility staffing experience within the last three years.
5. Current BLS card.
6. Successful completion of screening tests with a score of 70% or greater.

RESPONSIBILITIES:

1. Utilize the nursing process to assess, plan, implement, and evaluate patient care.
2. Perform focused interviews to identify specific patient needs.
3. Assess signs and symptoms indicating physiological changes in the patient’s condition.
4. Collect, analyze, and interpret data from healthcare team members and document actual and/or potential nursing diagnoses.
5. Document the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
6. Identify and prioritize actions based on patient care requirements.
7. Monitor consistency between the patient plan of care and actions of medical staff.
8. Perform interventions according to identified priorities, plan of care, and hospital policies and patient care outcome standards.
9. Provide a safe, comfortable, therapeutic environment for patients.
10. Initiate and evaluate patient and/or family education.
11. Revise the plan of care based on evaluation, changes in medical plans, and effectiveness/ineffectiveness of interventions.
12. Document the patient's response to nursing and other interventions.
13. Use clinical judgment in evaluation activities to meet patient care needs on the assigned unit/floor, including establishing priorities.
14. Monitor the safety and effectiveness of the environment and equipment.
15. Participate in activities that facilitate good interpersonal communication with the chosen facility.
