Licensed Social Worker

We are looking for a compassionate and dedicated Licensed Social Worker who will be responsible for providing support and assistance to individuals and families in need. The ideal candidate will have a deep understanding of social work principles and practices, as well as excellent communication skills and the ability to build relationships with clients. 

Responsibilities:

• Conduct assessments to determine the needs of clients

• Develop and implement individualized treatment plans

• Provide counseling and support services to individuals and families

• Connect clients with community resources and services

• Maintain accurate and up-to-date client records

• Collaborate with other healthcare professionals to provide comprehensive care

• Attend meetings and trainings as required

• Participate in quality improvement initiatives

 • Bachelor's or Master's degree in Social Work
