Licensed Social Worker, Hospice

As a Social Worker with us, you will provide unwavering emotional support to patients and their families during these challenging times. Your role will be essential in facilitating difficult conversations and addressing fears, hopes, and wishes with sensitivity and understanding, guiding individuals and families through this important transition.

We are dedicated to creating meaningful and lasting memories through unique end-of-life programs like the Gift of a Day initiative. By working closely with an interdisciplinary care team, we ensure that every patient and family receives the necessary resources for comfort and well-being.

Licensed Social Worker Qualifications:

• Licensed Medical Social Worker
• Master’s or Bachelor’s degree completed in a program accredited by the Council of Social Work Education
• 2 years of experience working in a medical setting with experience with hospice and/or palliative care
• Ability to work in both hospice & palliative programs
• Provide emotional, social, and grief support to patients and family members, collaborating with spiritual and bereavement support staff.
• Participate in Interdisciplinary Group meetings, identifying social-psychological issues and their interrelatedness with medical problems.
• Assist in coordination and participation in Ultimate Gift Programs, such as Gift of a Day and Veteran Recognitions.
• Reliable personal vehicle/transportation, valid driver’s license, and ability to safely operate said vehicle to visit patients and perform job duties within the field.

Licensed Social Worker Schedule & Benefits:

• Schedule: Monday to Friday, 8:30 AM - 5:00 PM
