About the Role

ATC Healthcare Services is seeking a qualified and compassionate Social Worker holding a LSW Social Worker certification to provide support services in an Adult Day Care setting. The Social Worker will play a key role in enhancing participants’ emotional well-being, coordinating care, and supporting families while ensuring compliance with state regulations.

Key Responsibilities

• 
  

  Conduct psychosocial assessments and develop individualized service plans for adult day care participants.

  
  


• 
  

  Provide counseling, emotional support, and crisis intervention as needed.

  
  


• 
  

  Coordinate with healthcare providers, administrators, caregivers, and community resources to ensure continuity of care.

  
  


• 
  

  Assist participants and families with social services referrals, benefits, and community programs.

  
  


• 
  

  Maintain accurate and timely documentation and case records in compliance with state and facility requirements.

  
  


• 
  

  Participate in interdisciplinary team meetings and care planning.

  
  


• 
  

  Advocate for participants’ rights, dignity, and quality of life.

  
  


• 
  

  Ensure compliance with HIPAA and all applicable regulations.

  
  

Qualifications

• 
  

  Current New Jersey certification/license as LSW Social Worker (required).

  
  


• 
  

  Minimum 3 year of experience in social work, healthcare, senior care, or community-based services preferred.

  
  


• 
  

  Strong interpersonal, communication, and organizational skills.

  
  


• 
  

  Ability to work collaboratively with multidisciplinary teams.

  
  


• 
  

  Experience working with older adults or special needs populations is a plus.
