Job Summary
We are looking for a qualified Therapist to join our team! You will play a crucial role in evaluating patients and developing individual treatment plans in collaboration with patients’ physicians. Operating with professional expertise and deep care for patients, you are a natural problem solver and self-starter. You enjoy working in fast-paced environments that afford you the autonomy to bring your best.
 
Team Leader

The ACT Team Leader is an integral member of the Assertive Community Treatment (ACT) Team, providing comprehensive mental health and dual diagnosis services for individuals who have serious and persistent mental illness and have not connected with traditional mental health outpatient clinics. The ACT Team Leader works with clients to develop client-driven goals and creative ways to help clients meet these goals. The ACT Team Leader provides cross training to other team members and helps ensure that all team members are aware of and focused on helping clients work on their recovery goals, including wellness and transition goals. As a team leader, you will be the contact point for all team members, so your communication skills should be excellent. You should also be able to act proactively to ensure smooth team operations and effective collaboration.

Principal Duties and Responsibilities

  • Direct the day to day operations of the ACT team including scheduling staff to ensure appropriate coverage for day, evening, weekend, holiday shifts, and on-call hours.

  • Direct and coordinate treatment, rehabilitation, and support services in collaboration with the psychiatrist.

  • Oversee the administrative operations of the team.

  • Provide clinical oversight of services ensuring patients are seen in accordance with their needs and the delivery of best and ethical practices. 

  • Conduct clinical assessments and provide direct clinical treatment, rehabilitation, and support services to consumers as needed.

  • Coordinate and supervise admission, assessment, and treatment plan development.

  • Facilitate and monitor discharge planning in collaboration with consumer, family, psychiatrist, team members, and relevant outside requirements.

  • Establish effective relationships with consumers, families, treatment team members, and designated government and managed care officials.

  • Provide clinical and administrative supervision for the ACT team to include but not limited to: facilitating clinical supervision meetings, treatment team meetings, coordinating training for team and completing evaluations and function as a practicing clinician on the ACT team

  • Perform on- call duty for crisis intervention, staff supervision and consultation.

  • Oversee management of medical records ensuring compliance with company policy, Medicaid and other third-party requirements.

  • Participate in staff recruitment and selection process.

  • Develop and conduct staff orientation and training.

  • Performs other duties as required.  



Education and Experience: Master’s degree in a relevant discipline and with appropriate licensure or certification to provide clinical supervision to the ACT team staff, plus a minimum of two years’ post-degree clinical experience with adults who have serious mental illness is required. The ACT team leader is a Qualified Mental Health Professional (QMHP) or Mental Health Professional (MHP). The ACT team leader also provides direct services to beneficiaries in the community within their scope of practice. 

Qualifications

  • Master’s degree in social work 

  • Minimum one year of experience in health care and social work  

  • Current CPR certification 

  • Valid driver’s license 

  • Psych experience is a plus
