The Licensed Vocational Nurse provides direct general medical and psychiatric nursing care to clients, including completion of all facility-designated assessment instruments assigned to the LVN, collection and documentation of assessment data, development, review, and updating of client individual service plans (ISPs) within the facility's level of care, implementation of physician orders and ISPs, medication administration, prescribed treatments, monitoring, documentation, and timely communication of changes in condition. Assessment data and ISPs requiring Registered Nurse review are submitted for RN clinical review, analysis, concurrence or direction for revision, and signature in accordance with facility policy and applicable requirements.
All medical and psychiatric care, treatment decisions, and medication regimens are directed by the treating physicians. The LVN practices under the direction of a licensed physician or Registered Nurse.
