We are seeking a compassionate and dedicated Licensed Vocational Nurse to join our team. The LVN will be responsible for providing direct patient care under the supervision of a Registered Nurse. The ideal candidate will be able to follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes. The LVN will also respect and promote the mission, values, and vision of the company.
Requirements
Primary Job Duties:
Provides direct patient as defined in State Nurse Act.
Implements current nursing practice following a patient assessment and an approved plan of care initiated by Physician/Nurse Practitioner.
Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
Provide accurate and timely documentation of patient services to reflect the plan of care.
Participates in coordination of Palliative Care services, appropriately reporting the identified needs for Palliative Care interdisciplinary group members
Perform basic health screenings
Record and assess clients’ symptoms and health related complaints
Document client’s medications, treatment and condition in charts
Bring all significant questions, concerns and complaints to the attention of the counselors and/or administrative staff
Complete all applicable paperwork i.e. medical logs, shift reports
Perform home visits
Case management
Performs other duties as assigned by the Case Manager and Palliative Care Lead.
Participates in personal, professional growth and development. Also participates in organization's quality management program.
Other duties as assigned by senior management
Qualifications
Qualifications include:
Education: Graduate of an accredited Licensed Vocational Nurse (LVN) program
Current Basic Life Support (BLS) certification for Healthcare Providers sponsored by the American Heart Association.
Skills/Experience:
Completion of an accredited LVN program in California
Current state driver’s license
Unlimited access to a reliable car
Two (2+) or more years hospice or palliative care experience preferred
Excellent communication and interpersonal skills
Demonstrated knowledge about home care, hospice, palliative care, advance directives, pain management, symptoms control, and discharge planning
Preferred English and Spanish speaking
Preferred experience with electronic health records
Working conditions
Normal clinical office environment. Occasional overtime may be required and/or hours may be shortened as business needs dictate.
Physical requirements
Requires both sitting and standing for long periods at a time. Due to the nature of nursing there are periods of time where heavy lifting may be necessary. Occasionally manual labor may be required. The physical requirements of this position may evolve as duties change.
