Job Description - Licensed Vocational Nurse (LVN) - Community Health (CalAIM ECM Program)

Job Type: Full-time‬

‭ Pay:‬‭ $30.00 - $35.00 per hour‬

‭ Expected Hours:‬‭ 40 per week

Work Location: Hybrid remote in Santa Clara, California

Currently, we are looking for a Licensed Vocational Nurse (LVN) to join our Community Health team as part of the CalAIM ECM Program. In this dynamic role, you will play a crucial part in delivering essential healthcare services to underserved populations, ensuring that every individual receives the attention and support they deserve. Your ability to work collaboratively with a dedicated team and your passion for improving community health will be vital in making a positive impact. As an LVN, you will engage in various nursing duties, contribute to the development and implementation of care plans, and work closely with clients to promote their overall mental and physical health. Responsibilities

  • Provide direct nursing care to clients in community settings
  • Assist in the assessment and evaluation of clients' health needs
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Monitor and document clients' health status and progress
  • Educate clients and their families on healthcare, wellness, and preventive measures
  • Coordinate with multidisciplinary teams to ensure a holistic approach to care
  • Support clients in accessing mental health and social support services

  • MUST reside in Santa Clara
  • Current and valid LVN license in the state of practice
  • Minimum of 1 year experience in a healthcare or community health setting
  • Strong understanding of mental health care principles and practices
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team-oriented environment
  • Compassionate and patient-centered approach to care
  • Willingness to travel within the community as needed
