FULL TIME QUALITY ASSURANCE NURSE (LVN)
Overview
Active Hospice Care is proudly serving the patients and families of Los Angeles county and surrounded areas for OVER 15 YEARS. We are currently offering Full Time or Part time Office Position for qualified Licensed Vocational Nurse (LVN) to join our dedicated team. Our goal is to make sure our patients, their families, and caregivers receive exceptional care and support throughout their Hospice journey.
Duties and Responsibilities
- Review clinical records to assure appropriate documentation. This review includes verification of appropriate documentation, quality of care provided, visits utilization, adherence to the care plan, and evidence of communication between disciplines.
- Oversee the maintenance of clinical records and files to comply with Conditions of Participation (COPs).
- Audit charts for clinical oversight and compliance and adherence to the Medicare/Medi-Cal guidelines.
- Manage clinical activities, including patient assessments, care plan development, service level determination, on-site field visits, and the implementation/coordination/maintenance/evaluation of care plans.
- Ensure service quality and participates in care coordination to ensure proper communication between, patients and caregivers.
- Conduct and/or delegate the assessment and reassessment of patients, including updating of care plans and interpreting patient needs.
- Coordinate communication between team members/physicians/caregivers to ensure the appropriateness of care and outcome planning and ensures compliance through review of documentation and care coordination activities.
- Must be able and willing to travel locally to visit patients when necessary.
Requirements:
• Current LVN license in California.
• Minimum one year experience in Hospice or Home Health.
• Demonstrated understanding of Hospice philosophy.
• Excellent written and verbal communication skills.
• Valid driver's license, auto insurance, and reliable vehicle.

Typical care starts with a team of Physicians, Registered Nurses, Social Workers, Chaplains and CNAs who provide highly specialized end of life care. Clients can expect regularly scheduled home visits by team members to evaluate the patient’s condition, provide symptom management, address ongoing problems, and supervise care and comfort. Utilizing the interdisciplinary care team approach the hospice helps to educate and instruct family members or primary care providers on how to manage physical and emotional symptoms such as pain, anxiety or confusion.
Other hospice services can include assistance with the personal care of the patient such as bathing and feeding; assistance with household chores; transportation to and from the inpatient unit, supplies, equipment and medications related to the primary illness, emotional support and counseling for both patient and family; spiritual counseling; short-term care for problems that cannot be managed at home and follow up bereavement counseling for family members after the patient’s terminal event.
