Life Transformation Coach / Fatherhood Case Manager

CPSEMO is seeking 1 full time dedicated Life Transformation Coach (Case Manager) who will be responsible for assessing client needs and developing, monitoring, and evaluating participant’s Individual Service Plan (ISP) and progress in the healthy marriage and responsible fatherhood initiative program.  Each Case Manager will maintain a caseload of 30 participants. Case managers will provide 

1. A holistic assessment of the participant’s strengths and needs and development of an ISP;


2. The implementation of the ISP (i.e. coordinating community resources and working with the participant to meet identified goals).
   
   
   
   1. The case managers will develop an ISP for each participant no later than twenty (20) calendar days after the participant’s orientation.  At a minimum, the ISP will address each of the six (6) following core services:
      
      
      
      • Parenting skills development;
      
      
      • Effective co-parenting with the child’s guardian;
      
      
      • Employment;
      
      
      • Education;
      
      
      • Child support; and
      
      
      • Domestic violence.
      
      
      
      
   
   
   2. The case managers will identify individual needs and goals, as well as strategies to achieve the goals.  The goals will be specific, measurable, achievable, realistic, and time framed.
   
   
   3. Case managers will make modifications to the ISP as goals are met and, if applicable, as additional needs arise; review the ISP regularly to assess whether goals have been met or need to be changed; and continue to follow-up to ensure that the participant has received requested services. 
   
   
   
   

Case managers will make contact with each assigned participant at least once every two (2) weeks.   If the case manager has not made either verbal or written contact with the participant, after four (4) weeks, the contractor shall exit the participant from the program. 

Case managers will connect clients to services that address immediate needs related to housing, substance abuse, physical and mental well-being, violent or risky behavior, and other areas that threaten to inhibit full participation and ability to achieve personal and healthy marriage and responsible fatherhood initiative goals. The case manager will document each meeting with a participant with written progress notes.  Progress notes will present an assessment of the situation/father, including strengths observed, an objective account of the interaction, and a plan for meeting the specific goals listed in the ISP.

Specific experience required:

Programs that serves fathers: 1 year- Prior case management experience and or prior work with fatherhood/families programs

Other: 1 year- Good writing skills, good work ethic, willing to work as a team player, Valid record keeping, follow up with client population.

Bachelor degree in Social Work preferred but not required.
