Description of the role:

University Home Care is seeking a Licensed Master Social Worker (LMSW) for our location in Livonia, MI. The LMSW will be responsible for providing comprehensive social work services to patients in their homes.

Responsibilities:

• Conduct initial patient assessments and develop personalized care plans


• Provide counseling and support to patients and their families


• Coordinate with healthcare providers to ensure holistic care for patients


• Advocate on behalf of patients to access necessary resources


• Maintain accurate and up-to-date patient records

Requirements:

• Valid LMSW license in the state of Michigan


• Prior experience in home care or healthcare setting preferred


• Excellent communication and interpersonal skills


• Ability to work independently and as part of a team
