LMSW Case Manager

Case Manager, LMSW - sign on bonus eligible, up to $4k

Arms Acres is looking for a Clinical Case Manager to join our inpatient team in Carmel, NY. In this role, you will provide comprehensive clinical case management services that includes assessments, individual, group and family treatment, treatment plan coordination, referent relations, discharge planning, and intervention with client support systems, including social service programs and managed care.

Requirements

REQUIRED QUALIFICATIONS:

• LMSW, LCSW required
• Master's Degree in related field
• Experience working specifically with substance abuse counseling preferred
• BLS must be completed within 3 months of hire
