Payrate: $28-$30/hr

Key Responsibilities

• Deliver nursing care to residents, including assistance with activities of daily living (ADLs).
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document patients’ vital signs, health status, and responses to treatment.
• Collaborate with RNs and other healthcare team members to create and implement individualized care plans.
• Provide education and support to residents and their families regarding health issues and care processes.
• Maintain a safe, clean, and respectful environment for patients and staff.
• Comply with all facility policies and procedures while providing care in accordance with established standards.

• Valid LPN license in the state of New York.
• Graduation from an accredited practical nursing program.
• Experience in long-term care or similar healthcare settings preferred.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with a multidisciplinary team.
• Strong organizational and time-management skills.
• Basic Life Support (BLS) certification is required.
• Flexibility to work varying shifts, including evenings and weekends as needed.
