Position Summary
The LPN will support the Nurse Outreach Program by providing field-based care and assessments to individuals experiencing homelessness, ensuring quality and compassionate health services across NYC.
Key Responsibilities
• Assist RNs with field health assessments
• Provide basic care, including wound dressing and vitals checks
• Support documentation and patient interaction
Required Qualifications
• Active LPN license in NYC
• Experience with vulnerable or homeless populations
• Ability to work outdoors and in transit
