We are looking for a Highly motivated, experienced and compassionate LPN ready to utilize theoretical knowledge and practical skills gained to provide exceptional patient care and contribute to the success of our health care team.
RESPONSIBILITIES: -Assist to oversee our agency medical department for Individuals with Developmental Disabilities who has medical challenges -Ensures medications are administered with zero medication errors -Conduct daily patient assessment, schedule medical appointments and follow up. -Collaborate with interdisciplinary teams, including physicians, therapists and direct support staff to achieve optimal patients' outcomes. -Train Direct Support Professional to care for the individuals served by the agency. -Participates in team training sessions and workshops, enhancing team members knowledge and skills. -Contribute to decrease in hospital readmissions by ensuring the individuals receive appropriate follow up care and support after discharge -Adhered to medication schedules, ensuring timely administration of medications to individuals -Respond promptly to emergencies, initiating appropriate actions taken to ensure quality care for our individuals. -Computer literate and comfortable in scheduling individuals' appointments as needed.
