LIFE (Living Independence for the Elderly) is a Medicare/Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. At an Albright LIFE location, you’ll work with a team of health care professionals providing exceptional, coordinated services each day that help participants stay healthy and safely live independently at home.

Albright LIFE has a mindset to help others and is a certified Great Place to Work where you can build a purposeful career. Let’s do all the good we can in this world – together!

Once you've submitted your application, you'll receive an email and text invitation from our trusted partner, Arena, to complete a short 3-5 minute questionnaire. Completing this step is required as part of the application process and helps us get to know you better.

Job Description

  • Collaborate with the care plan team to assess and address residents' physical, social, nutritional, spiritual, and activity-related needs.
  • Perform home visits with participants as needed.
  • Coordinate home care schedule.

Qualifications

  • One year of experience working with a frail or elderly population.
  • Active Pennsylvania Licensed Practical Nurse (LPN) License.
