Job Summary:


We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health and Hospice team. In this role, you will provide high-quality nursing care to patients in their homes, following the physician’s treatment plan and under the supervision of a Registered Nurse. All care is delivered in accordance with agency policies and within the LPN scope of practice as defined by the State Practice Act.



Essential Duties and Responsibilities:



  • Deliver skilled nursing care to patients and their families, as assigned and within the LPN scope of practice.

  • Complete accurate and timely documentation for all assigned patients and promptly report significant findings to the RN Case Manager.

  • Support the patient’s overall well-being by promoting a safe, clean, and comfortable environment, ensuring proper nutrition, rest, and hygiene.

  • Participate in specialized training related to nursing skills required for Home Health and Hospice care.

  • Provide coverage for nursing visits across the agency’s geographic service area, as needed.

  • Participate in on-call rotations for Hospice and Home Health patients, in accordance with LPN scope of practice, with RN support available.

  • Perform additional duties as assigned.



Minimum Qualifications:



  • Required:

    • Current LPN license in the State of Montana

    • Valid driver’s license, reliable transportation, and appropriate auto insurance

    • Access to high-speed internet for remote work and documentation



  • Preferred:

    • 1–2 years of LPN experience in acute care, home health, or long-term care

    • Proficiency in clinical skills such as aseptic technique, wound care, catheterization, ostomy care, and bowel programs
