Job Title: LPN Department: Home Health and Hospice Positions Available Employment Type: Full-Time
Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health and Hospice team. In this role, you will provide high-quality nursing care to patients in their homes, following the physician’s treatment plan and under the supervision of a Registered Nurse. All care is delivered in accordance with agency policies and within the LPN scope of practice as defined by the State Practice Act.
Essential Duties and Responsibilities:
Deliver skilled nursing care to patients and their families, as assigned and within the LPN scope of practice.
Complete accurate and timely documentation for all assigned patients and promptly report significant findings to the RN Case Manager.
Support the patient’s overall well-being by promoting a safe, clean, and comfortable environment, ensuring proper nutrition, rest, and hygiene.
Participate in specialized training related to nursing skills required for Home Health and Hospice care.
Provide coverage for nursing visits across the agency’s geographic service area, as needed.
Participate in on-call rotations for Hospice and Home Health patients, in accordance with LPN scope of practice, with RN support available.
Perform additional duties as assigned.
Minimum Qualifications:
Required:
Current LPN license in the State of Montana
Valid driver’s license, reliable transportation, and appropriate auto insurance
Access to high-speed internet for remote work and documentation
Preferred:
1–2 years of LPN experience in acute care, home health, or long-term care
Proficiency in clinical skills such as aseptic technique, wound care, catheterization, ostomy care, and bowel programs
