LPN

Job Summary:
The LPN at Heart of the Carolinas Home Care will provide high quality and compassionate home care services to patients in Greenville,Greer South Carolina. The LPN will work as an individual contributor and must have a valid license as either an LPN in the state of South Carolina.

As a FT/PT employee at Heart of the Carolinas Home Care, the LPN will receive a competitive salary based on experience and qualifications.

Responsibilities:
- Provide direct patient care in the client's home under the supervision of a registered nurse
- Administer medications and treatments as prescribed by the physician
- Monitor and document patient's progress and response to treatments
- Communicate effectively with physicians, patients, and their families to ensure the best possible care
- Follow all state and federal regulations, as well as company policies and procedures
- Participate in mandatory in-services

Requirements:
- Valid LPN license in the state of South Carolina
- Minimum of 1 year of experience in a healthcare/medical setting
- Excellent clinical skills and knowledge of current nursing practices
- Strong communication and interpersonal skills
- Ability to work independently and collaboratively in a team environment
- Compassionate, patient-focused, and reliable
- Must pass a background check 
- Valid driver's license and reliable transportation
