Brief overview of Responsibilities:

Assist the Registered Nurse in carrying out the plan of care for each patient
Assist the Registered Nurse in performing specialized procedures as directed by the Registered Nurse under physician's orders.
Assist with the preparation, implementation, and continuing evaluation of the patient care plan.
Recognize and understand the effects of social and economic problems upon patients and provide for the emotional and physical comfort and safety of patients and their families.
Help teach our patients and their families appropriate self-care techniques.
Active participation in team and staff conferences to ensure well-coordinated care for patients.
Prepare clinical and progress notes based on patient visits.
Observe, record, and report to the appropriate person patient symptoms, reactions, and changes, as appropriate.
Participation in a weekend on call rotation
Other duties as assigned

Qualifications:

Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia
At least one year of hospice experience required.
Graduate of an accredited nursing school.
Must have a valid driver's license and a reliable car available for use during work hours.
