LPN - Admissions Support

As an LPN in our support team, you’ll play a key role in bridging clinical care and compassionate coordination. This position is responsible for handling DME ordering, E-kit preparation for new admissions, and EMR documentation to ensure a seamless onboarding process for patients. You’ll work closely with hospitals to facilitate discharge planning and enroll patients into care, serving as the trusted face of our hospice team during this critical transition. Your attention to detail and ability to collaborate across teams will help ensure that every patient and family experiences a smooth, supportive introduction to hospice care.

LPN Admissions Support Qualifications & Responsibilities:

• Active Unencumbered state LPN License
• 1 year of experience in an environment collaborating with multiple disciplines and exposed to end of life care
• Excellent verbal and written communication skills
• Current unencumbered driver’s license and auto liability coverage
• Experience with discharge planning, DME and EMR documentation
• Ability to shift priority focus based on needs of the admissions team.
• Detail oriented
