LPN - Care Coordinator

Keystone Health is seeking a LPN Care Coordinator to join the Enabling Services team.

This position is full-time, Monday - Friday (8:00 am - 4:30 pm)

The Care Coordinator collaborates closely with RN Care Managers and providers to support a defined panel of patients enrolled in the Chronic Care Management (CCM) program. This role focuses on facilitating patient goal setting, providing virtual clinical support, and enhancing overall patient outcomes and quality of life. Care Coordinators serve as a central point of contact and an educational resource for patients, families, and the care team.

QUALIFICATIONS:

EDUCATION and/or EXPERIENCE: Current Licensed Practical Nurse license.  Knowledge of Patient Centered Medical Home initiative or willingness to learn.  Minimum of two (2) years working in a clinical setting.  Experience in care management desired.  

LICENSE/CERTIFICATION(S):  Valid LPN license. Current CPR certification.
