LPN Care Manager - Hiring Immediately

• The LPN Care Manager position is a driving position and will require travel throughout Franklin County.
• This is a Full-Time position, 8:30am-4:30pm, with 1 weekend day rotation every other month.
• We provide superior care and superior services to patients at their end-of-life journey. Only those who have a heart for hospice will succeed.

The LPN Care Manager's Essential Duties:

We provide superior care for patients in families in homes/facilities. The Care Manager plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. You will effectively interact with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.
